Summer 2023 playscheme booking form for 4 to 8 year olds

Dates: 21st-25th August 2023
Ages: 4 – 8 year olds
Who can attend: The playscheme is open to all young people in Leeds with learning disabilities or additional needs who 4-8 years old.
Venue: The Vinery Centre, 20 Vinery Terrace, Leeds, LS9 9LU
Times: Start at 10:00am and finish at 3:00pm (3:30pm finish on the day of the trip)
Cost: £20 per day per child and an additional fee of £20 on the Thursday to help cover the cost of the trip. (Please only pay once you receive confirmation of your place from Leeds Mencap)

How to request a place for your child
Please complete and return a Leeds Mencap playscheme application form as well as a booking form.
You also need to complete the below booking form requesting which dates you would like your child to attend. Please be aware that if we are oversubscribed we may not be able to offer you all the dates that you have requested.
After the closing date we will let you know which dates you have been allocated and how you can pay for your place. Please do not send payment until you receive confirmation from Leeds Mencap that your child has a place on the playscheme.
